Hello friends ....i m proud to say that my case is being published in RH'S OFFICIAL JOURNAL'AUDE SAPERE' which is edited by Dr.Ashok mohanty sir.
CASE:
This is a case of a male , 50yrs who worked as a cook in a canteen of a Company.
Presenting c/o
Pustular eruptions on buttocks , associated with pain & burning since 2 yr. He had been to skin specialists but got temporary relief with some antibiotics but eruptions again used to appear after stopping the treatment.
He was wearing a dirty shirt & ‘lungi ‘. Apperaed very calm & quiet . I asked him some questions regarding his pain & burning to which he replied in a very low voice.
When I asked him to show the eruptions. He got up very slowly & showed me the eruptions. He pressed the area surrouding the pustules.The area was swollen & pus was coming out badly. He took some pus & blood on his finger by pressing the area & show it to me. He said’DOCTOR SAAB YE DEKHO AISA HAI’. After showing it to me he cleaned his fingers by rubbing it on his shirt itself. & without thinking much he sat in the chair ( I noticed afterwards that the chair on which he sat was full of pus )
I : ‘what do feel regarding this problem? ( ye taklif ko leke kya chinta hoti hai?)

Patient: dard aur jalan bohot hota hai . aur kuch nahi( pain & burning is there, nothing else)

I : aap kaise sahan karate ho taklif ko ?. kam karate waqt koi badha nahi hoti?

Patient: nahi aise hi sahan karata hu. Hamara kam khana banana ka hai . Canteen mai bohot garami hoti hai , fir aur taklif hoti hai. Lekin phir bhi kam karate rehata hu.

I tried to ask him many questions so as to understand his mental state but couldnot get much.
Only thing I could get is , he is very mild person who don’t want to mix with people & want to be busy with his work. He also told me that he never like to oppose even if some one tries to disturb him. He said’ Mai kisise ZAGADA nahi karata . Agar koi muze pareshan kare to mai usake saath phir kabhi baat nahi karata .Mai bas aapane kam se kam rakhata hu’. During the whole conversation he was very serious & never smiled . Even when I tried to crack some joke he remained calm as if he has nothing to do with it.

With this information I gave him ARS.ALB 30 single dose…….& ask him to come after a week.

After a week when he came ,there was no change in the pustules . They were smelling badly. He again took some pus & blood on his hand by pressing the area surrounding the pustules & showed it to me . This time he cleaned it on back of the chair on which he was sitting. I was very angry with this & asked him ‘ye kya kar diya?’. To which he just gave an expression as if he felt guilty & kept looking downwards but did not say anything”. This time also he was wearing the same old shirt & ‘lungi’ with the stains of pus & blood on it.

I asked him ‘ aap khana banate ho aur itana ganda(dirty) kaise rehate ho’. To which he said nothing & kept looking downwards.

Now I could get some symptoms which I missed on first visit.
- Dirtiness, everything;dirtying
- Elegance , want of elegance
- Smiling , never
- Reserved
- Talk indisposed to, desire to remain silent

I gave him a dose of AMM.CARB 30 & SL for 7 days

After a week when he came , I could see the improvement as there was no pus in the eruptions even after pressing it hard. Slight blood was oozing from it. Pain & burning was much much better than before.

I repeated 2 doses of Amm. Carb 30 as per the requirement .Patient is still under the follow up & is fine

A case of DIABETES MELLITUS

49 YR OLD MALE---- RIKSHA DRIVER-- came to me with reports of BSL ( F-283, PP- 301), Urine sugar ( F ++,PP +++)

PATIENT : Mereb dost ne kaha ki aap achhi dava dete ho ,isliye aapake pass chala aaya . Muze 3 din hi pata chala ye report mai ki muze diabets hai'.

I :aapako kya taklif hai?

PT: muze badan mai bohot dard hota hai, furti kam lagati hai, jodo mai dard hai, aur doctor saab KAMJORI bohot hoti hai. Bhuk achhi hai. Sab kuch khata hu , lekin agar aap mana karoge to sab parheg karlunga. Sirf kal mutton aur daru pi leta hu , fir paraso se hum treatment chalu karenge.

I: aur kya taklif hoti hai?

PT: doctor sahab .. aisa lagata hai ki takat nahi hai, body patali ho rahi hai.

I: urine ki koi samasya?

pt: bohot baar jana padata hai.

Through out the case taking ,he was very cheerful, laughing , joking.

pt: doctar sahab kya mai 'dokala' kha sakata hu? ....to which i replied 'yes" again he asked me kya mai fruits kha sakata hu........i said 'yes' lekin mango aur chikoo mat khao. He again asked sab log bolate hai ki karela aur jamun khao .........kya karu 'khalu?'...........to replied ........kha sakate hai.

I asked 'why u dont want to take allopathy medicine?

pt said.....mera ek dost hai ..unko bhi diabetes hai ......unko aapaki dava se fayada hai isliye mai bhi yahi dava khana chahata hu. Also he asked me kya mai exercise kar sakata hu?........i said ...definetely...to which he replied ..mai to har subhah nange pav roj daudata hu.........i asked 'why barefooted? ...he said mera padosi bola ki agar nange pav daudoge to DIABETES jaldi thik ho jayega. I told her not to run like that ..to which he said ..'THIK HAI...AAP BOLATE HAI TO AISEHI KARUNGA'.....

I explained him the seriousness of taking medicines & disease........& prescribe him

BAR. CARB 6 ......1 DOSE & PLACEBO FOR 7 DAYS.
